
2008 Tackle Diabetes Program - Indianapolis Colts
Kids and family members—Do you want to learn how to manage type 1 diabetes with an easy, online tool and participate in fun activities with the Indianapolis Colts?
By joining the team to “Tackle Diabetes” with the Colts, Cerner, Riley Hospital for Children, Lilly Diabetes and Roche Diagnostics, you’ll accomplish all this and more!
Here is how the program works:
- All patients will have access to the online tool after they pick up their registration packets.
- Registration packets can be picked up at the kick-off event or either of the open house events listed below. If you can’t make the kick off event, you can contact Riley Hospital for Children at 317.274.3889. Please choose option “Tackle Diabetes” in the phone mail.
- Each packet will contain a How to Get Started guide and a step-by-step guide on how to use the online tool.
Here are the program requirements:
- Check your blood sugar four times a day.
- Upload your glucose results (or self enter) to your online record.
- Submit your glucose meter readings to your clinicians at Riley Hospital for Children once a week.
- Join us at all the fun events we have planned this season!
Incentive program overview:
We want to encourage you to Tackle Diabetes by regularly checking your blood sugar and using the online diabetes management tool. As a reward for managing your condition, we’ve teamed up to offer some great prizes and fun events.
- Attend the kick-off at the Conrad in downtown Indianapolis with the Colts. There will be food, games and pictures with the Super Bowl Trophy
- Get autographs and pictures with Colts players, cheerleaders and Blue the mascot
- Win great Colts merchandise
- Win tickets to a Colts game where you will get to watch the game from a private suite

Mark your calendars now for these upcoming events, open to all children with type 1 diabetes and their families. Make sure you RSVP for each event by sending an email to cernerdiabetes@cerner.com. Additional information will be provided at the Kickoff party on how to attend the other events:

- Tuesday, September 2 from 6-8 p.m. (The Conrad Hotel)
- Open house events with players, cheerleads and mascot
- Wednesday, October 15 from 6-8 p.m. (Clarian North Hospital)
- Tuesday, November 18 from 6-8 p.m. (Riley Outpatient Center-Downtown Indianapolis)
Game day viewing party-Indianapolis Convention Center (For every child who follows the program requirements)
- Sunday, December 28 starting (TBD depending upon NFL Flex Scheduling)
- Every child who follows the program requirements listed above will be invited to a game day viewing party with food, fun activities and prizes.
- Everyone who attends will have their name put into a drawing, and 24 lucky winners will be escorted into the game to watch from a private suite!
- For those who do not win suite tickets, a private tailgate will be held at the Convention Center to watch the Colts take on the Tennessee Titans. Enjoy food, raffles an appearance by Blue and the Colts Cheerleaders!
