Due to the expected influx of COVID-19 cases, Disaster/Overflow beds will need to be activated. Bed Coordinators will need to understand bed availability of negative pressure rooms along with potential cohorting options for positive COVID-19 results. In order to effectively manage room turnover, Cerner recommends reviewing your milestone priority settings to ensure cleans will be prioritized accordingly.

Clients can use existing functionality within CareAware Connect to aid in care coordination and communication. Dynamic Role profiles can be utilized to aid in the identification of and communication with key roles such as Infectious Disease, House Supervisor. Speed Dial entries should be reviewed to ensure critical, static contacts are readily accessible, such as Lab, Pharmacy, Security. Event Routing and Millennium Discern Alert integration should be considered to distribute result notifications that can aid in expediting isolation protocols and bed placement.

By utilizing lab results associated with COVID-19, an association of the result to an existing Airborne or Droplet isolation indicator can increase awareness. Clients can also create new Isolation indicators associated with the Lab results.

Cerner Patient Observer
Clients can use existing functionality/hardware of Cerner Patient Observer to limit unnecessary exposure of staff as well as conserve PPE supplies. By utilizing the camera and 2-way communication a staff member would be able to view a patient or quarantine area without being present. CPO allows you to maintain real-time contact with a COVID-19 population while limiting exposure. When the appliance is on a rolling cart it allows you to continue virtual observation care across your organization. Place cameras at intake areas to replace greeter or be able to provide a preliminary triage without undo exposure to staff. Additionally, utilize CPO cameras in areas that have not been used as patient care areas previously may require power and network.

Soarian Clinicals
Clients can use existing Soarian Clinicals functionality to assist with operational readiness and communication in responding to the need for surge capacity, staff and volunteer training, and communicating patient specific information related to COVID-19 status.
Healthcare organizations throughout the country are exploring options to increase their bed capacity. Clients may access the link below for a recommendation page on setting up healthcare units as well as beds. It is important to also refer to information from your patient access system in addition to the information provided in this quick reference guide.
The communication of patient conditions, such as isolation and COVID-19 status, can be accomplished with model visit and patient level indicators, as well as eighteen client definable patient indicators. These indicators can be updated directly in Soarian Clinicals, a transaction from your patient access system, as well as via Workflow Engine processes. The patient indicators are visible throughout Soarian Clinicals to aid in interdisciplinary communication.
To support the training of temporary staff and volunteers in the use of Soarian Clinicals, we have created a uLearn site to allow access, without a uLearn license and log on, to a library of clinical workflow topics. For those using our purchasing application, the Project Track functionality can be utilized to monitor spending for supplies related to COVID-19. For those using Surgical Management or Point of Use, supply usage can be reported on and transmitted to a foreign materials system.

The World Health Organization has published guidelines on what diagnosis codes to use for COVID-19. Acute Care
Cerner released enhancements to Millennium to help clients effectively screen and monitor patient populations. It’s encouraged to immediately incorporate the latest update in the Infectious Disease Travel Screen PowerForm within Millennium. See FLASH20-0073-3 for more information.
Both the World Health Organization (WHO) and Centers for Disease Control and Prevention (CDC) recommend enhanced transmission precautions for patients with Coronavirus Disease 2019 (COVID-19). Therefore, Cerner recommends that clients use Enhanced Droplet and Contact precautions for these patients to indicate additional measures are required. Additionally, we recommend that COVID-19 be added as a reason for isolation.
COVID-19 Worklist: Clients can use existing functionality within the Worklist Framework product solutions to aid in patient screening and tracking, and care coordination. This worklist will leverage the latest Infectious Disease content to alert the clinician of patients’ recent travel or infectious disease screening. The worklist will also provide clinical data within the columns to aid in screening and monitoring patient population.
Cerner recommends implementing a rule that populates a problem to the problem list when a COVID-19 test is ordered, and a rule to update the problem when the result is positive.

Acute Case Management
With Cerner Acute Case Management, clients can utilize existing functionality to document COVID-19 within the clinical documentation improvement, utilization management, and discharge planning processes. Ambulatory
Cerner’s Ambulatory Strategy team has published configuration and Model Experience workflows for screening and monitoring COVID-19 patients. Clients can leverage PowerForms to document patients’ recent travel or contact risk screening precautions (see FLASH20-0073-3 for more information). Health Maintenance Recommendations can be leveraged to monitor patients at risk or confirmed for COVID-19. Population management or optional patient outreach can be achieved leveraging Dynamic Worklists and/or Invitations. Active or passive clinician alerting is available via SmartZone and/or Discern Alerts for patients who have been confirmed positive for COVID-19.
Cerner’s Quick Visits product, available with the standard MPages release, supports the grouping of items that are commonly ordered and/or documented as part of routine patient visits. A clinician can use these groupings to quickly complete visits that are performed on a regular basis. To help contribute to our Model Experience related to COVID-19, Cerner is making content available through the Quick Visits solution to provide clinicians a single place to chart items for routine visits. Cardiovascular
Cerner released enhancements to Cerner ECG Management and Cerner Cardiovascular Imaging Management to help clients effectively screen and monitor patient populations in dealing with COVID-19. Isolation Status can now be easily viewed in the Patient Profile of the Cardiovascular Workflow Manager (CVWFM). QTc discrete data can now be viewed to track significant changes.
Long-Term Care
Cerner released enhancements to Millennium to help clients effectively screen and monitor patient populations. It's encouraged to immediately incorporate the latest update in the Infectious Disease Risk Screen PowerForm within Millennium. See FLASH20-0073-1 for more information.
Cerner's Long-Term Care Strategy team has published information regarding our Model Experience related to Coronavirus (COVID-19) and the waivers being issued for Skilled Nursing Facilities by CMS. Recommendations for Skilled Nursing Facility (SNF) patients being admitted under the waivers of the 3-day hospitalization, PASRR, and physical environment requirements, monitoring for respiratory illness, and telemedicine visit documentation are included. Recommendations for a COVID-19 Worklist to aid in patient screening and tracking, and care coordination is also included. CareTracker recommendations are also available for screening and tracking.
Interoperability
If adopting the latest Infectious Disease Travel Screen PowerForm, update your C-CDA template configuration to include recorded data in the Functional Status section of the documents and ensure the appropriate COVID-19 results are configured to populate the Laboratory (Results) section. C-CDA configuration guidance can be found HERE.
Completing these steps will allow your organization to exchange this information with other care providers through avenues such as Direct Messaging, CommonWell, Resonance, and other exchanges.
If needing to push patient data to a city, county, or state health departments or a regional collaboration; a modified version of the Transition of Care (TOC) operations job (ops job) can be configured. The modified TOC ops job can be configured to qualify patients based on COVID-19 or related results to send out a CCD/CCDA document to a specified direct address. Emergency Medicine
Existing functionality within Cerner Emergency Medicine solutions aid in patient screening and tracking, care coordination, and result follow up related to COVID-19.
At the point of entry, PowerForms document patients’ recent travel or infectious disease screening, which immediately triggers the appropriate isolation precautions order and tracking event. In addition, the problem alert column in ED LaunchPoint can display an icon when a COVID-19 problem is documented.
If your Emergency Department(s) are creating Field EDs, we recommend creating a specific Field ED tracking list or zone, to effectively manage patients outside the regular ED and avoid exposure.

Perioperative
During the Preadmission Testing and/or Preoperative Intake process, PowerForms can be used to document patients’ recent travel or infectious disease screening, which immediately triggers the appropriate isolation precautions order. Subsequently, an Isolation event can be manually set on the Perioperative Tracking board to further draw attention to isolation patients. Additionally, a summary of the COVID-19 airway recommendations as defined by Anesthesia Patient Safety Foundation (APSF) and the American Society of Anesthesiologists (ASA) has been condensed into a single anesthesia action, which can be developed locally across Cerner's client base.

Cerner content developers and the Care Pathways team have published initial risk assessment content based on CDC guidelines, along with instructions for how to import the content and build the Care Pathway within the client domain.Through the use of the Care Pathways build tool, this content can be quickly modified to meet the rapid changes in recommendations related to COVID-19. In addition to communicable disease screening, Provider Documentation content includes a Sepsis Screen as well as a Sepsis Care checklist. Demographics Banner Bar
The coronavirus disease 2019 (COVID-19) demographics bar was developed to give clinicians a face-up indicator of a patient's COVID-19 status. Users can quickly and easily view if an order has been placed or view the patient's test result if available. With this enhancement, the COVID-19 demograpahics bar looks across the patient's orders and encounters to return the latest documented result. If the most recent result is collected within the same hour of another result, the higher critical result will display. Four different statuses are displayed based on the status of the COVID-19 order. In addition to the result associated with the order, if no order exists but a previously documented result does, then the COVID-19 field displays the previously documented result.
Laboratory
Cerner’s Laboratory team has published information regarding our Model Experience related to Coronavirus SARS-CoV-2 (COVID-19) for test configuration (including test naming conventions) and Model workflows for monitoring pending and in-process tests. The team published a standard report using Discern Analytics 2.0 for your laboratory staff to monitor the incidence of positive and negative COVID-19 test results, and the team also published a standard Discern Expert rule to notify personnel at your organization via email when a positive COVID-19 result is published.
