BayCare Health System, a leading not-for-profit health care system in central Florida, is leveraging advanced technologies and services from Cerner, Lumeris and Salesforce to better manage its populations. BayCare’s overall strategy to increase access to care and improve the health status of its patients is supported by a combination of industry-leading solutions from these three organizations. Clinicians are using population health strategies and additional capabilities to extend care beyond the exam room and improve health outcomes, inclusive of the health system’s senior patients participating in its Medicare Advantage plan.

Foundational to BayCare’s approach is leveraging Cerner’s HealtheIntent® population health platform, Salesforce’s CRM technologies and Lumeris’ value-based care services and operational expertise. BayCare is collaborating with all three to identify and solve for gaps in care, as well as enhance consumer and provider engagement to improve health outcomes for its populations.

BayCare is using Cerner’s big data and insights platform for population health management to analyze medical data across platforms and support clinicians in achieving actionable insights to help engage people beyond the hospital setting and manage health outcomes. A single view of an individual’s health experiences, by aggregating clinical data in near-real time from multiple sources, supports BayCare clinicians with decision support to guide treatment and care. BayCare’s population health management approach, supported by Cerner, emphasizes providing high-quality and proactive care to engage patients and improve care.

Salesforce supports BayCare to collaborate more efficiently, better understand their patients and build stronger relationships between the patient and the entire care team, with the aim of higher consumer satisfaction and improved health outcomes. With Salesforce, BayCare can personalize communications at scale to deliver pertinent information at the appropriate time to support engagement and care plan adherence goals.

BayCare is working to provide an optimal patient experience and better outcomes for seniors enrolled in the BayCarePlus Medicare Advantage (MA) health plan through their strategic operating partner, Lumeris. BayCare selected Lumeris based on the company’s extensive experience working with providers to develop, operationalize and execute on value-based care strategies, including MA plans. Lumeris has a proven track record of supporting improved outcomes, engaging physicians and helping increase patient satisfaction. BayCarePlus encourages providers to focus on well-being, rather than traditional episodic care, and supports increased access for seniors.
